How to Request for Globe SIM Replacement?
The process for getting a new Globe SIM replacement is very easy. Just visit the nearest Globe Store and apply for a SIM replacement. They will provide you with a form—fill it out and submit it. Your request will be processed within 24 hours.
It is mandatory that you are the owner of the SIM and have already completed the Globe SIM registration or TM SIM registration; otherwise, your request will be canceled. You can apply for a replacement for various reasons, such as a defective SIM, a lost SIM, or upgrading to a 5G SIM.

Step 1: Bring all the necessary documents based on your SIM replacement reason.
Step 2: Visit the nearest Globe Store.
Step 3: Request a new SIM replacement and provide the reason.
Step 4: Fill out the form and pay a ₱50 fee.
Step 5: Submit the form and wait for your new SIM to arrive.

Requirements for SIM Replacement
You are required to bring the following documents for SIM replacement:
- Physical SIM/eSIM voucher.
- Notarized Special Power of Attorney (SPA) (if applicable).
- Two government-issued IDs under which the old SIM was registered.
- Customer Acknowledgment Form (if deactivating a lost/stolen SIM).
- SIM bed/GCash Card/eSIM voucher (if available).
- A clear selfie (head to shoulders, eyes open, not blurred).
